Immunizations
From the Parent/Student Handbook
Students must show proof of immunization. A student who does not comply with the immunization requirements will not be permitted to continue in school. Students with medical conditions or sincerely held religious beliefs which do not allow immunizations must complete a waiver statement or affidavit. Forms are available in school health offices.
Unimmunized students may be excluded from school in the event of a disease outbreak.

Additional Immunization Information:
To attend school, children in Nebraska are required to be immunized against the following diseases: • Diphtheria, tetanus, and pertussis • Polio • Measles, mumps, and rubella • Hepatitis B • Varicella (chicken pox)
Additional Note: One Tdap (tetanus, diphtheria, acellular pertussis) is required for entry to 7th grade.

****A student may be provisionally enrolled in school if he/she has begun the immunizations against the specified disease prior to enrollment and continues the necessary immunizations as rapidly as is medically feasible. ****
